I find it rather hard to believe that there are that many unpatched SQL Servers without the relevant ports blocked at the firewall. From first hand (naive) experience, it takes a day or so to get infected by Slammer - it is still in the wild.
When you do get infected you'll know right away: your LAN switch will light up like a flashing Christmas tree and your Internet access will be effectively blocked... or may in fact be blocked by your ISP if they're any use.
