Most outages...
...in my company are due to Cisco bugs. Its gotten to the point that Cisco happily sold us a pile of gear so *we* the customer can regression test IOS.
It takes far too much time and effort to upgrade a Cisco product. No matter how much you test, you will miss something and there will be a bug for it, and management will ask us why we didn't test for that too.
Cisco has a great thing going! Compile the code straight to their website and the let the customers sort out the bugs!
